I Have a Written Certification From a Medical Cannabis Practitioner, Now What Do I Do? 🩺

You just finished your screening with a medical cannabis practitioner in Kentucky but you don’t know what to do next. Here’s exactly what to do to get your medical cannabis card in Kentucky:

Start Your Application

Once you have received the written certification from an authorized practitioner you must apply for your medical card.

To apply for your medical card you can click here to be redirected to the registration page.

Once you are logged into the portal select “+Create New Application” and “Patient” from the drop down menu to open a “New Patient Registration”.

For caregivers, select “Caregiver” and “New Caregiver Registration”.

How to Submit Your Application

In-state Patients

Retrieve your written certification by clicking “View Available Certifications” on the “Practitioner Written Certification Information” tab.

After you retrieve your certification submit a digital copy of your signed and notarized signature page on the Documents tab.

Minors need to do the same thing, but submit this form instead.

Caregivers

Once your patient is approved you can submit a registration application.

Make sure to sign and submit this form.

Receiving Your Medical Registry Card

Approved cardholders will receive a digital registry ID card within 30 days.

Your card will be accessible through the portal under the licenses tab.

You can download and print a physical copy of your card as well as possess the card digitally.
